I cannot stress enough how important it is for you to get SUPPORT. Go to http://www.lalecheleague.org/WebUS.html and find a leader near you. CALL them. Ask them to be with you in the hospital. The lactation consultants are good in the hospital, too, but you don't get to take the lactation consultants home. La Leche League leaders are available 24 hours a day, any time that you have a problem. You should try to start going to meetings NOW, so that you can get to know the leaders and be comfortable with them. They can walk you through everything. Breastfeeding is not always easy, but it IS always the best thing for your baby. It is an incredibly rewarding experience to breastfeed a child, knowing that you are giving that child the very best start to his or her life possible. But lots of women quit or give up because of two reasons: 1) lack of information (they don't have any idea how inferior formula is, or how good for your baby breastfeeding is -- they think formula is just as good and it ISN'T), and 2) lack of support (they have a problem, don't know how to fix it, and it's easy to just open up a can of formula and ruin that delicate balance that your body has developed). You can get BOTH of those things from LLL. But you don't want to wait until you have problems latching your baby on before you start trying to find help, have it ready NOW.
By the way, you absolutely can and WILL have enough milk for your baby. Nature sees to it that you do, or else we as humans wouldn't have survived for as long as we have before formula was invented. Your body knows what your baby needs, as long as you don't confuse it by giving your baby formula or other supplements. About pumps: be careful. Your body knows how much milk to make based on how much the baby nurses. You can really mess up your supply by pumping and giving the milk to your baby in a bottle. Plus, you lose a lot of the benefits of nursing by putting the milk in the bottle (sound crazy? Listen to this: when your baby comes in contact with a germ that can cause an illness, s/he passes it onto you while breastfeeding. Then, your body makes the antibody for it and passes that to your baby so that neither one of you gets sick from the germ. Can't do that with bottles!!)
And crazy dreams are normal when you are pregnant!.
Congratulations! You should be so proud to have made the choice to breastfeed.
P.S. - breastfeeding shouldn't ever hurt. I've seen a lot of people saying that it hurts. If it hurts, the baby isn't latched on properly and you need help fixing that problem before it gets worse!

Preparing for Breastfeeding: Women who are planning to breastfeed do not need to do anything to prepare their nipples for breastfeeding during pregnancy. Expressing fluids from the breast manually before delivery may lead to an infection of the breast (mastitis) or even early labor. The body prepares the areola and nipple for breastfeeding by secreting a lubricant to protect the surface. This lubricant should not be rubbed off. Observing and talking with women who have breastfed successfully may be instructive and encouraging.

As the mother eases the nipple and areola into the baby's mouth, she makes sure the nipple is centered, which helps keep the nipple from becoming sore. Before removing the baby from the breast, the mother breaks the suction by inserting her finger into the baby's mouth and gently pressing the baby's chin down. Sore nipples result from poor positioning and are easier to prevent than to cure.

I had the same problem, and my daughter at 2 weeks was not back up to her birthweight, so I bought a breat pump and pumped enough breastmilk to feed her. With a pump, you can see how much the baby is eating and be sure that you are feeding your little one enough. Ask your doc about pumps. Many hospitals have them for rent far cheaper than you can buy them.
But, try it naturally first. :)

I won't lie to you, that fear probably won't go away. My son was 20lbs at 4 months which off the charts big and I still had that fear. But all moms have irrational fears.
It is easy to tell if your baby is getting enough, you just count diapers.
Also please join La Leche League, hopefully the one near your is full of kind souls who won't look down on you because of your age, as I do not. However, I obviously can not guarantee that. Find support wherever you can. http://www.llli.org
