In the anime, Envy was created by Hohenheim in an attempt to resurrect the son he and Dante had, who had died from mercury poisoning when he was eighteen years old.

In the manga, he is described as the fourth homunculi to be created by Father.

In the Anime, it was Hoheimeim Elric, Edward's dad. In the manga it was the leader of the homunculi, a man known to us as "Father".
In the anime, homunculi were created by means of human transmutation, but in the (more brilliant) manga, the Homunculi were created by placing a philosopher's stone inside a host body, and if the body did not reject the stone, the human would evovle into a homunculi. If the stone did no accept the body and/or the soul, the poor human would die a very cruel and painful death. All the homunculi were made this way, in the manga.

In the anime, Wrath was created by Izumi, Sloth by Ed and Al, Envy by Hohenheim, Greed by Dante(I think?), Lust by Scar's brother, and Gluttony and Pride were not mentioned, :D

It was Hohenhiem Elric (the father to Edward and Alphonse Elric) who created Envy. It was 400 years before Ed and Al and all other characters except Hohenhiem and his lover Dante were born, at that time Dante got pregnant and the child belonged to Hohenhiem but unfortunately the child died right after his birth, so Hohenhiem tried a human transmulation to bring his son back again but like what Ed and Al and Izumi have done Hohenhiem had failed to and what he had created became Envy.
