at the 12 week appointment
u need to carry yr notes with u all the time incase u need medical attention!
bring this to yr healthcare provider's attention asap - even if it means calling yr gp and/or midwife now!
it is even advisable to take yr notes with you on holiday as they carry the records for your baby and your pregnancy upto and including yr last appointment (blood, urine, pressure, etc) which are vital if you need medical attention

I got my notes about 15 weeks and had to carry them everywhere with me. If you want to keep your notes after you have given birth then you have to pay them £50 and they will copy them for you.
