Well, if this person is your bestfriend, then I'm sure you know their passions, things they enjoy most and dreams. Take those and use them in the gift. A few examples my friends and I have done for each other are:
a helicopter ride, skydiving for two, cooking classes, pole dancing classes, a room makeover, a kidnapping (woke them up in bed, blindfolded them, took them to a local spot for a surprise picinic with friends and family)... use your imagination and surprise them with something that takes some thought. Even if you don't have alot of money, small things you know they would appreciate - try baking a homemade cake (even if it's crap it shows you put more effort into it, more than by just getting a gift certificate to the mall! - unless they REALLY want that!!! lol!!!) My best friend collections first addition books, by specific authors, have bought several for her, she also reads intently, have bought her several books, that I know she enjoys.
Since she is my best friend I know what she really enjoys, does not have to be an expensice item, just something from the heart.

before one of my g/fs was deployed to afghanistan last year, for her 22nd birthday..i compiled movies and music pics...from our childhood years...up till she flew out...she got to watch them while she was in flight ...i got an email from her week later...it was the present she will always cherish, and she watched them every chance she got, help her relax...ooooh memories..

nothing much...cuz they didnt buy things for me.....but sometime I gave them chocolate bars and even mechanical pencils
