I guess what you are asking about is the first release by a band that was recognized as being 'punk' at the time it was released, rather than a historical interpretation of something that is looked back on as being a forerunner of punk.
The first record that people recognized as being punk at the time it was released is the 'New Rose' single by the British punk band The Damned. It beat the Sex Pistols ('Anarchy in the UK') by a few days.
The Ramones (USA) first album came out earlier, and the Ramones are certainly the forefathers of the British punks, but the Ramones weren't yet considered 'punk' since the label had not yet been applied to the music. (BTW: You can find the Ramones first demo on-line, it's called 'Loudmouth'.)
If you want to get into historical reinterpretation, then you can go back to Iggy and the Stooges, the MC5, the myriad of garage bands in the 60's and even Link Wray in the 50's. But the record that suddenly had people saying 'that's punk' is 'New Rose'.

The term punk rock was apparently coined by rock critic Dave Marsh in a 1970 issue of Creem, where he used it to describe the sound and attitude of ? and the Mysterians. The first real Punk acts were Patti Smith, The Ramones and The Sex Pistols.
