How are decisions in real world organizations actually made?

By the "olde Buddy system"---you do for me and I'll do for you, should we can't do it, hire a mutual friend and expect a kick back away from the office--"feathers of the same stick together". This will end when the justice/legal system views white collar crime more than just a nuisance. and put the perpetrators in jail for keeps, not a few week ends and a fine less than the criminals cost for coffee.

In an effective, active, forward-thinking organization, decisions are made by committee, who offer a concensus to the leader. the leader either accepts the concensus decision, rejects it and asks for a follow-up or flies by the seat of his pants. If one flies too often, the job is given to someone else (unless one is terribly lucky!) A team in a company is based upon gathering a group of diverse people, with diverse knowledge, to bridge the required operation. No one person can know all there is go know about a situation; a concensus offers the maximum information available in the least amount of time.
In all the other types of organizations, usually one person at the top makes the decision, based upon knowledge (experience or assumed), hunches or power of position. Most successful companies follow the first path; governments, non-profits, hospitals, doctor's offices and schools follow the second.
