I think Dr. Grey would be played by Jennifer Aniston.. Dr. Sheppard by George Clooney.. Dr. Burke would be Denzel Washington.. Dr. Stevens- Reese Witherspoon.. Dr. Sloan- Orlando Bloom.. Dr. Karev- Josh Harnett.. Dr. Bailey- Whoopi Goldberg.. Dr. Torres- Demi Moore.. and well the rest.. Im not sure of.. this one made me think :)

It would be a very difficult cast to replace...they are all great as their characters, but here's my go at it: Dr. Sheppard--George Clooney (I just can't argue with that one), Dr. Burke--Djimon Hounsou (yummy), Dr. Montgomery--Sandra Bullock, Dr. Grey--Cameron Diaz, Dr. Yang--Lucy Liu, Dr. Stevens--Scarlett Johansen, Dr. Bailey--Jennifer Hudson (Yeah she's a little young, but could pull it off), Dr.Karev--Ryan Reynolds, Dr. O'Malley--Paul Walker, Dr.Sloan--Matthew McConaughey, Dr.Webber--Morgan Freeman, Dr.Torres--Eva Mendes. That was a fun one!!

To begin with they wouldn't change the cast and Ellen Pompeo, Patrick Dempsey, Sandra Oh, and Catherine Hiegl have all made movies not for television.
But you want to waste money on big names, so

Of course you will have to leverage the whole studio to pay for the cast.
