Dear (name of employer, if addressed to a person),
You would recall that I worked with you from (date) to (date). It was a pleasure to have worked with you in the company.
I have a small request to make to you. There is an amount owing to me from the company from the superannuation fund. If there is a standard form for claiming this amount, please do send it to me. I will return it to you for processing the claim.
I am in the process of applying for another job.You know that a prospective employer would normally make a reference to the previous employer about the applicant. I cant think of another better person than you for this purpose.
Would it be alright if I gave your name as a reference for this purpose? I am sure that knowing my work, you wouldnt hesitate to answer such a reference. In case you have any hesitation in this regard, do please let me know soon. I can then ask someone alse.
You have my phone number in your diary, and so please feel free to call me whenever you want.
Thanks for your help.
Yours sincerely,

I think it depends on how well you knew/know the person on how formal the letter should be.

If you are writing to Sir/Madam or Whom it Concerns then sign off with Yours Faithfully.

Make is concise. Use short sentences... and good grammar and correct spelling

I would add all my contacts at the top of the letter to save anyone having to look up those details.

I would contact the person by phone if that was possible first; then confirm request by letter.
