I'm thinking it started when Twiggy (a super-skinny model in the 60's) became a superstar model. She was so extremely skinny, and since, we've gone to somewhere in between her body style and Marilyn Monroe's.

I believe it's due to runway models. In the 60's, Twiggy started this, and then later on, the image was followed up by supermodel Kate Moss (and others). Many designers think their clothes look better on these ill-looking women and there have been recent attempts to boycott these shows. Unfortunately, this trend is causing many young women to become bulimic/anorexic because they think that's what's popular. However, it's most unhealthy and dangerous. And besides, if they would just ask their male counterparts, most guys prefer curves.

There are a number of causes but I believe a significant one is the fashion / catwalk industry.
Designers are lazy and want to just drape some fabric - And in all fairness, you can put anything on a skinny person and use clips to make it look tailored.
For normal women, they would have to actually tailor each garment, the clips trick would not work.
And Hollywood does mimic the fashion industry.
But that is just one of the reasons . . .
