Currently the Prime Minister designate is chosen by members of the party having the greatest representation in the House of Commons. Current custom now is that Her Majesty summons the Prime Minister Designate and invites him or her to form a Government. As Prime Minister, head of Government, has a weekly audience with Her Majesty in which the Prime Minister reports, answers questions, and receives "suggestions" from Her Majesty. Her Majesty is Head of State.

Not quite sure how a person becomes President of the USA; I guess it is whoever has the most money to spend on the campaign. I think the President does report to the people from time to time, telling them whatever he wants them to hear or think. And the President is Head of State, I think.

NEITHER are Royalty; neither are Royal. However some Presidents act like they are; perhaps some Prime Ministers try also, for a while.

When the president of the USA is elected.It is by popular vote of all voters of that country.The electoral college has some input.Once this person is inaugurated,they have almost unlimited power.This person appoints an inner cabinet of whomever they choose.Almost like medieval kings
The british prime minister is the nominated leader of his political party He too has an inner cabinet but all of them must be elected members of parliament.IE answerable to the people.Plus under the Westminster system if the ruling party is defeated on any motion by the opposition.The prime minister must seek the monarchs' permission to dissolve parliament and call an early election.
