I missed some key deadlines and my boss is angry. My written job performance was very bad. I did complete many assignments, but not the key ones.

COMMUNICATION my dear.
Firstly, were you just LAZY or were you too swamped with work that you didn't have time? Why didn't you prioritize to do the Key assignments first? (think about that, I'm not ragging on you, this is important)...
I do not know if your job review was done WITH your boss, supposing it was, I don't know what you said... did you make excuses? Did you have any good reasoning? What was their reaction?
Now obviously from the beginning you needed to have better communication with your boss, let them know that you couldn't finish these for whatever reason so it could either be delegated or your boss could've told you in which order to do it in so you would've done the KEY ones first. Its not about effort, or what you DID, fact is, in the real world- you have to prioritize... and sometimes **** doesn't get done... you just gotta make sure its the unimportant ****... and if its all important, well, then you gotta make time for it. Or you just shouldn't be in the job you have.
If you know that you made a mistake (and you DID make a mistake, no one but you is responsible for your assigned work)... you should tell your boss. Request to have a meeting with him/her to discuss your job review- Start by telling them you know what you did wrong (the too many smoke breaks? leaving early, coming in late? chit chatting instead of working hard? or you were too swamped but didn't prioritize correctly? you could've stayed late or worked a weekend but choose not to?) Tell them you know what you did wrong and you are sorry and you promise to improve your performance and to work hard on it. Request suggestions and advise with whatever your mistakes were, and DO THEM. Take notes during the discussion, then actually DO AS They SUGGEST... go ahead and communicate where you don't think you can do it or where you foresee difficulties. Then before you leave, request another meeting with him/her to discuss progress in approximately 2 weeks from now. Make sure you write down exactly what you have done inbetween your first meeting and your second. What you did different and at your second meeting ask how else you could improve. You just have to BUST A** to prove your worth.
Put aside your pride... its hard to say "I messed up. I was lazy. I want to fix it and prove I can do better." and then, actually do better... They won't can you. I don't know what your job is, but in any field, its a b*tch to train someone. It takes time and man hours and trial and error... but you need communicate this regret and explain your mistakes and then fix them.
Alot of times our personal lives may seem more important then work... thing is, we work to live... and you need your job. So you're there for 8 hours a day, POUR YOURSELF into work for 8 hours, then put it away. Thats all they ask of you. 8 hours x 5 days a week. Just as work isn't a priority on the weekends... personal life isn't a priority during the work day. Thats just how it goes. Until you are in a cushy position where you don't have to worry about being Canned... take that approach... and you will not fail.

You mention that you missed some key deadlines... and your performance was bad. So could the deadlines have been met if you had help ? In other words should YOU have asked for help ?

Maybe there is too much on your plate - I don't know, but delegating some of your work out may be in the cards.

Meet with your boss and let him know that you (sincerely) want to improve. If there is too much on your plate and you can't handle it - let him know. That does not mean you are a failure... it means you need help.

If the bad review is the result of you "just not doing your job"... then I would suggest getting out.

Time to pony up and shine like a star. That's the only reason you get a bad review - your boss is pointing out what you did wrong so you can get it right!! I am a boss - no one wants to fire people.

if you know what you did wrong, work hard to get it right. make sure you get all your assignments turned in by deadline.
