First runner-up is only used when there is a second runner-up as well.
Ex. in the Geo Bee, you have two class winners and a first runner up and a second runner up.

The terms are interchangable. "Runner up" is "1st runner up". "1st runner up just tells which of the runner up positions it was, even though that is redundant.
