Very rare, but I can't say that I have not used the 5 second rule when it was something I really wanted to eat and I trusted where it fell. Generally I will just throw it away or offer it to my cats or any other animal that may want it and is around.

All of my food gets dropped outdoors and I have to pick it up and eat it, and no the five second rule does not apply outdoors. =)

i've had arguments bout this and it's a three sec rule outside or on particulary icky ground, but no, definitely not, as some non-important person said, ewww

No I'm to weak stomached. I do good just to eat let along if it falls to the ground. I don't think I even done it as a child.
