The only silly questing is the one that isn't asked.

Ideally, you cleanse your vulva with a wipe before you begin to urinate for that collection. That way, no, a doctor generally can't see any signs of any type of sexual activity in your urine. If you suffered a small tear, there might be a bit of blood in your urine, but that would be as individual cells and not visible to the naked eye. The same is true, by the way, if you have your period, even if you are using a tampon.

If you've having a pelvic exam, though, and in particular if you're having a pap smear, you do need to abstain for a day or two before that exam.

Urine is the byproduct of anything you have put into your body...therefore if you did drugs (alcohol, cigs, cocaine, etc) then yes it would be in your urine...even if you have since excreted some of it, your system will continue to flush out things for a longer period of time than just a day.

If you are concerned about passing a drug test knowing you did drugs the night before...well they can probably see it

If its a regular urinalisis then they will not find anything weird. If they choose to do a microscopic test then they could find sperm but not drugs and other stuff. Drugs are found only with specific tests. That needs to be sent to the lab and specified before hand.
