I have a swordtail whos belly looks a bit sunken, but its really hard to tell if thats just its normal body shape. Im afraid it may be internal parasites. Should I only treat for it if im sure it has them, or should I be sure first?
And if I should treat, what medicine is best?

Well the thing is, she is in with other fish so I cant just look at the poop in the gravel, also its very hard to catch them pooping. This is why im not sure. If I feed medicated food, is this gonna effect my other fish? I can set up my quarintine tank if I need to

The only treatment for internal parasites is medicated food. Diagnosis by poop is the best way to tell, weird as it may seem. If the fish has normal brown poop then she probably does not have internal parasites. If it is white then she probably does.

Yes you should but if u find a treatment for other things to just in case but it should work just as powerful on internal parasites
