It is a short form of the word 'continued'. Sometimes the abbreviation used is 'cont.' or 'contd.'.
The example quoted would be on forms which ask for the address of someone, where the space allotted for the address may be inadequate if the address is a bit long and cannot be filled in within the first or second line. Here more than one line or two lines might be provided for the full address to be written as a continuation of the earlier lines.
