The most efficient and successful way to improve one's communication skills is to 'COMMUNICATE'.

Yes! Unless one is not getting into the practice of communicating all the time, it is hard to get it.

Feel free to communicate (in the language you desired to develop)! Never hesitate for the pronunciation mistakes or of its grammar.
But when you did those mistakes or may be blunders(depends on the language), try to avoid the next time and for ever.
Gradually, try to communicate nicely, error-free and fluently.

Its the straight-way path to master any language.
Best Wishes!

Well, you do not say where you live. Your first language was learned by hearing it in your infancy and gradually, eventually mimicking the flow of sounds and meaning to get what you wanted as a child. Only then did you begin to connect the lines on paper formed to become words and complete thoughts and eventually to the print in a book and the computer screen. That was a lot of practice, practice, practice. And of course, learning from mistakes. Repeat that process as an adult? Well, yes! Listen, listen, listen. And read the comics in the newspaper - then ask about the meaning and discuss it with an English speaking person - have some fun and do not stress out about the grades in a classroom setting. I know people who can make an A+ on a test of English, but cannot achieve the goal of fluency you seek. Relax and have some fun with it...make it a "sport" if that motivates you.

the best and easiest way u can learn proper english( written and then spoken english gradually) is by reading english papers daily.

make it a regular habit.

read papers like hindu which are well known for perfect english.

read english books. to start with let it be small easy to follow magazines or books. slowly u can go for bigger volumes.

always make sure u keep a dictionary handy and whenever in doubt about any word immeaditely check it u and then u will never forget the meaning.

start slowly conversing in english with simple sentences with your contacts.try it with ur near and dear who wud correct ur english if there r any mistakes and help u.

and most important be confident and its no big deal to speak perfect english. remember that nothing is impossible and u can do it.

hope wht i said would be of some help to you.

good luck. :-)

Talk in English, listen to the radio/watch TV/read - this will improve your vocab and, therefore, give you an opportunity to express your thoughts better and faster:-)
Also, practice in writing, because you need to have both speaking and writing skills.
