In short the secret to success is attitude. There's a difference between wealth and success and different degrees of each. Some people are successful without the need to be millionaires or billionaires and that's where attitude comes in. The main concept of success is to spend less than you earn, educate yourself as much as you can, consider your own business, save for the future, pay bills on time, and don't be afraid to give money away. They say when your hands are open to give, they are also open to receive. Your local library will have a number of good books that can help you plan your life and business as well as achieve your goals. The more goals you plan, write and achieve the more successful you will be.

I think it is just decisions people make on a daily bases.
Check this.
I'm 5'11" and 125 pounds. I'm very much under weight. However, if I want to maybe one day say become a "big buffy guy". I would have to not only write out a proper and healthy diet for my day to day, week to week, month to month, consumption.
I would then also have to plan out my work outs and such.
Then comes the REALLY hard part. Putting my plan into play. Because this is not a one time deal. It is a day to day choice.
Most people I know who are not that successful just simply do not give things much thought, planning, and or action. Thus the real reason. Not to mention people these days on average get depressed way to easily when one thing out of five goes wrong.

It's usually not a secret. Just read their biographies.
I think success comes in two parts: hard work, and luck. You can work hard to achieve your goals, but luck will get you in the door first.

In my opinion, there are several factors that contribute to a person's success in both business and in life. Here are a few of the ones that I, personally, hold important:
1) Be Persistent - if you give up easily or are discouraged by challenges you encounter along the way, you won't get very far. There are always people that are more eager and driven who will step up in your place if you do not. Life always throws us some curveballs, but take what you can from each hurdle and use it to move forward and hone your strategy as you go.
2) Be a Life-Long Learner - it's crucial to always be educating yourself about new things to continue to grow as a person. Also, with every new skill you acquire, you increase your chances of success and your competitive edge. Robert Kiyosaki, author of the Rich Dad, Poor Dad series, said in one of his books that if a person were to learn just one additional skill, their income would grow exponentially. Also, if you're looking to be successful in business, it's necessary to learn all you can about sales and marketing as, at its most basic, business is all about making the sale. Don't be afraid to ask questions for fear of looking "stupid". People love to be of help to someone who's learning or is not afraid to ask questions. This has been of enormous help to me in my career because I don't allow myself to be afraid of looking "bad" for seeking to enhance my knowledge.
3) Do Something You Love - It's so much easier to wake up every day and be excited to get to work when you're doing something you really love. Passion is a huge factor in success and if you love your work, it won't seem so much like work! Also, people are naturally attracted to those who demonstrate a passion for what they do. This also makes it easy to be a great salesperson of whatever you're selling. Enthusiasm is contagious!
4) Get a Mentor - no one knows everything coming out of the gate. If you can find someone to advise you along the way, to whom you can pose questions that are raised throughout your career, why not take advantage of the knowledge someone else has gained through their own trials and errors? Many people view giving back to others just starting out as a very important way of sharing their success.
5) Don't Be Afraid of the Competition - We can learn a lot from our competitors - both what they're doing right and wrong and we can, again, benefit from their trial and error. Also, isolating yourself for fear of revealing your secrets will be a lonely road. Get involved in your industry and see what others are doing. What opportunities can you discover to gain market share from seeing areas which need attention that the competitor might be missing?
6) Know a Little About a Lot - while the idea of specializing is very common, if your goal is to be an entrepreneur, it's actually more beneficial to learn the basics about as many areas of your field as you can. You don't have to know it all to be a success in business, you just have to know to surround yourself with people who can fill in the areas that are not your strongest suits. If you focus on the skills to which you are best-suited and bring on others - be it partners or employees - to fill roles that do not excite or interest you, you'll be happier and more successful.
Good luck!
