I never had bubbles until I started using this.

Don't worry it's normal. And yes it happens even when you follow the instructions. I have notice this in my tank too.

The issue is that everyone's water source is different and even it comes from their tap, the composition and pH is also different, so some people have less of this problem and other more. But it's has not cause problem for me so far.

The bubbles are caused when you add too much aquasafe. I had the same problem at first because I was using too much.

You're just using a little too much, it won't hurt anything. If you add only a couple of gallons, calculate from the directions on the bottle how much you need to add, it won't be much. If you add ten gallons of water, you can fill the lid top to the first line. If you're eyeballing it, it is easy to put in too much that way.

Every once in a while. I think I either put too much in or it reacted with my pH neutralizer. It has never bothered my fish.
