It means it seems like there's a lot going on but you are not getting anywhere. It's like when a car is stuck in the mud or on ice and it makes a lot of noise as you hit the gas and those wheels spin round and round, but you are still stuck.

You know how car wheels spin in the snow but you don't go anyplace? It's like that. It means you're doing a lot of work but not getting any results. Like explaining something to a stupid person who just doesn't get it.

"Spinning my wheels" but going nowhere.

I think of it the same as the following:

Grinding my gears.
Breaking my balls.
Driving me crazy.

Things like that, when a person annoys you.
