i do distinctly remember that there was one time i was actively controlling my dream; alas it was years ago and i don't remember the details.
i had prior to that had an interest dreaming (I had an issue of Omni magazine from the mid-80s with a how to guide I believe), but i know on this occasion i wasn't trying to do it.
i also had one dream where i did in fact die (in the dream) and the dream continued on for some time even after my death (so i was a third person observer to some degree after that point); someone shot me in the back with a shotgun and it was a strange sensation, but i dont remember what happened after i died.
I had other dreams where i must have awoken or something before my imminent demise (eg, i remember an F16 crashing down upon me as I stood on the ground, but I don't remember the impact)

yes. I personally believe that my dreams have a lot to do with the colors I see throughout the day. I know that the color blue makes me dream very vividly and I have dreams that seem like they last a lifetime because I can remember details so greatly from them. Colors like beige, gold, light orange and brown don't help me to remember any of my dreams. It sounds really weird I'm sure, but I say this because the walls in my room were once brownish-gold and used to have a hard time remembering my dreams and when I did remember my dreams they would be scary and I would always end up dead in them. When I painted my walls blue I noticed that I had been having dreams that were so real and lucid. I can remember details like never before. I even see colors that I rarely get to see in my dreams. These past couple of months with my blue walls, it is as though I go to bed just to play in a painting. Last night I even got to visit an aquerium/gallery. It was beautiful. To not be chased by dinosaurs, bulls and abnormally large silver worms is a relief. I can dream in the ocean for the rest of my life, especially since I live in a desert state. It's wonderful.

I'm assuming you're asking for drug- or meditation- induced dreaming. For me, neither has been the case. On rare occasions, I will sometimes, while still being in my dream state, realize that I am in a dream. With such knowledge, I know that I can do whatever I want, yet there is still a prevailing paranoia. As such, I will do something I wouldn't normally do in the situation, but not something so severe as to have extremely negative consequences. It's a fun feeling while it lasts.
But your question requested a description of "how it came to be," which I presume to be asking how I entered the lucid state. It just happens that way sometimes from my experience. There was nothing I did to initiate or enhance the process. I hope you find what you're looking for.

Yes it was amazing! It only happened a couple of times but it was so cool! What happened was, while I was in the dream I realized "Wait a second! I'm dreaming!" Once I realized that it wasn't real, then I could make the rules. I was in control. I wanted to fly. So I just jumped up & flew! It was awesome! I never wanted to wake up. In another dream I was being chased by a monster of some kind. All of a sudden I realized it was a dream so I turned to face my monster & it turned out it wasn't a monster at all. Then I decided to turn it instead to someone that I wanted to chase me! & then I didn't run away! It was very exciting! Once you are aware that it's a dream & you are in control then you can do anything, be anything. It's a very empowering feeling. If only I could have lucid dreams all the time! If only I could have super powers when I'm awake!

It has happened to me on several occasions, but not on a regular basis.
If you are interested in learning how to do it, start by writing a dream journal.
You have to get in the habit of writing your dreams down as they are fresh for you. The better you get at writing and remembering small details the better you will get at controlling what happens in the dream state.
