I am afraid it is either a bacterial infection or a viral infection, either way if this has been going on for a while it probably won't make it. take it to a vet and get it medication emidiatly

Its so hard to tell w/o knowing alot more like size of turtle and how long this has been going on for. There could be something missing in his diet. Try using a supplement like ''rep to vit'' by Hart. Or any other powder you sprinkle over their food. It could also be tempeture. Make sure when its cold that the turtle can warm itself somehow. This is more then likely your problem. Ill bet its getting too cold in his/her cage. good luck.

he probably has an upper respiratory infection (common in
turtles that get too cold and without the proper lighting) better
get to the vet before its too late

Yep. Vet. But do you have full spectrum lighting? What temp is the cage at? What type of setup do you have? etc. etc. yadda yadda yadda.........................
