One coffee of small size costs 280 yen, equal to about 2.3 USD, at Starbucks.

I think that 180 yen, about 1.5 USD, at DOU TOR Coffee shop is the cheapest. The next is 190 yen, 1.6 USD, at Saintmarc Cafe.

It costs around 350 yen, 2.9 USD, at an ordinary coffee shop.

The cost of a cup of coffee will vary depending on where you are located in Japan. The further you move from Tokyo (north or south) the prices tend to go up. Not much, but they do go up. Just a regular cup of coffee can cost anywhere from 150 to 250 yen. If you go into the gourmet coffee, you are looking at 300 to 500 yen, and that estimate is generous. But one good thing is that in most franchised places, you will get free refills.

Decaf is a whole other story. Even at Starbucks you will be luck to find decaf. (Just thought I'd add that.)

It depends on location, yes, but also if your visiting a kissaten, a Starbucks or similar, or especially how fashionable the location is. A standard cup of coffee will be cheaper outside Shiodome Station than it would be in Ginza or Harajuku.

With Starbucks and Excelsior exterminating all the local kissatens in Japan, the coffee at the smaller locations will generally be better, but can cost much more.
