If it means as much to you as my daughter's teddy bear means to her (it is also 23 years old) then why not take it to a professional doll restorer? Don't try and and do it yourself, you could cause more damage than good and end up with an even worse problem like taking it out to the trash.

with it being 23 years old you don't want to put him in the washing machine trust me. You will just find a mound of fluff. Get a wash cloth and some dawn with bleach. Just one small drop will do. Wet the cloth and a drop of dawn and just wash up. Repeat until clean. Let air dry.

no washing machine. with or without a pillow case etc! place in a large plastic bag with some baking soda, seal he bag and shake vigorously form time to time over serveral hours. brush the baking soda out of the fur with a stiff brush. If its really bad u mite need to do this serveral times but it comes out smelling clean and fluffly.

You can take him to a dry cleaner. They are basically professional cleaners. They will more than likely be able to help you with the cleaning process.
