Yes indeed, but also we should remember that it was the unmitigated arrogance of NASA that killed them.
All of the astronaut deaths have been due to the failure of NASA to place enough importance on the known facts.
NASA knew fire was a possibility for the three.
They knew the possibility of O ring failure.
They knew that tiles were falling off.
