The first time I went, I was very nervous, and I had a male doctor. But it really isn't that bad. Your doctor will just ask you some questions about your periods, and if you are sexually active or not, your medical history, etc. Then she will do the breast exam, which takes only a minute or two. Then she will do the pelvic exam and she will insert a speculum and a long q-tip looking object to scrap your cervix for cells. Then she will stick one to two fingers in your vagina and feel your uterus and ovaries. This all happens in less than five minutes. I highly doubt she will give you a rectal exam or an enema, you are way too young. You can have your mom in the room with you or a nurse. I'm not sure if a nurse has to be in there with you for a female doctor, but If you have a male then they do. Also, tell the doctor this is your first time, and they should explain everything that they are going to do.

It's perfectly normal to be afraid. To answer your questions:
1- It's kinda cold, as most doctor's offices are. They give you a paper gown and ask you to take off your shirt, pants and undies. You can leave your socks and bra on.
2- Before the examination is performed, the health care provider will discuss your menstrual, sexual, and contraceptive history with you. Then, you will be given a private room to remove your clothing and cover up with a paper sheet and vest. After your thyroid, breasts, and abdomen have been examined, you will be asked to lie down on the exam table, place your feet in foot rests at the end of the table and let your knees spread apart.
First, the external genitalia will be inspected visually. Next, a speculum (an instrument which allows visualization of the cervix) will be inserted into the vagina. You will have a sensation of fullness, but it should not be painful. Relaxing will make the exam more comfortable. You may bring your CD player or Ipod. Cells for a Pap smear are collected from the cervix. This can be an uncomfortable procedure, and sometimes mild cramping may occur. The specimen will be sent to a laboratory to be examined for signs of cancer or precancerous cells and other abnormalities.
After the speculum is removed, the health care provider will insert gloved fingers into the vagina and place his/her other hand on your abdomen, applying slight pressure. This pelvic exam is done to determine the size, shape, and position of the uterus and ovaries. Following this, a gloved finger may be inserted into the rectum to detect any abnormalities of the rectum, and to feel the back of the uterus and the ovaries.
The gynecological exam is not a test for sexually transmitted infections. If you do want to be tested, you will have to indicate that to your provider before your exam begins. The exam, however, is an excellent time to be tested and discuss sexually transmitted infections with your health care provider.
3- I've never had any shots during my gno exams.
4- Sorry, can't give you any details about the enema. Never had one - esp. not from my OBGYN.
The best thing you can do is relax. Good luck!

Whether you get a rectal depends more on the doctor than your age. I have been to 2 gynos and one did it, one didn't. Never had an enema, though. A rectal exam is basically the doc putting a finger up your butt and feeling around for anything unusual. Not the most fun but not the worst thing in the world. Feels like pooping backwards.

They shouldn't give you a rectal exam, I never had one...
They make you sit on a chair with stirrups to put your feet in so your legs are spread and they insert something into your vagina to open it up, then scrape a few cells off the vaginal wall. That's all. its uncomfortable but if you relax (hard i know, it being your first time!) it shouldn't hurt or bleed. No shots.
