to me, it would mean what special classes you have taken, what research you have done,,,, or any activities related to what you wish to specialize in,,,,,, for instance,, for pediatrics, you may of worked with children, researched different methods and styles of treatment on the internet,,,, etc , it would mean what have you done, to further your education in that area, that wasnt a requirement, either of your job or your schooling,,or your daily personal life,,,,
if you wish to be a cardiologist, and a parent had heart problems, learning about that would be a daily activity,,,,,,
