Team Canada was coined for the 1972 Summit Series with the USSR. That eventually became the Canadian Cup, then the World Cup of Hockey.
Both of these links are the Team Canada site and lots of history there.

i dont know, but what i do know is that it formed in WCW. Lance Storm started it when he first got into WCW-around 2000-2003 or something.
