For the same reason men dont go and regular yearly prostate exam when it too can save their lives.

Probably a combination of being uncomfortable and kinda painful as well as fear of knowing the truth if it comes out positive for cancer.

Multiple reasons.
1. Fear. Fear of the results, fear of the pain other women have told them about.
2. Cost. Not everyone has insurance.
3. There are a few mammo clinics that do overkill...everyone gets a mammogram, breast ultrasound and THERMOGRAM (which was a joke back in the 70's and still is), then convince the woman that she needs to come back twice a year. THEY need to be put out of business because they prey on women's fears.
4. Availability. Due to Congress' Deficit Reduction Act, reimbursement is low which is causing some mammo facilities to close.

Mammograms are controversial. Their quality depends all on the doctor. There are a lot of false-negative and false-positive readings and they are painful. I rely on checkups with my doctor (who is not ALL for mammograms) and for checking myself. If women go for them, great. If the RELY on them, big mistake.
