While you have the computer on just go over to word and note down a list of what exactly you need to do to make those amendments. Be specific in your list, eg, full read through and make notes to do with publishers' recommendations, re-write one section etc..
Then take a couple of deep breaths, turn off the TV, set up laptop and notes, make a cup of tea or coffee, then sit down and do the first task. Promise yourself you'll have a sandwich after you have done first task. Have some lunch, do the second task and reward yourself with a little break. Good luck.

I'm a writer and go through stages like that.What I tend to do is set myself a working schedule say 9 am to 11am where I'm not allowed to do anything else but work on my book.Brew some coffee,sit down and read 1 chapter.Read it twice then make any neccessary ammendments.Then take a break-set an alarm clock for in 15mins time and in that 15mins do whatever you want,but when that alarm goes off you have to sit back down and work on the next chapter.Keep doing that until 11am then take a complete break from the computer-no emailing,no yahoo answers-no computer! Walk aay from it,go find something to do that doesn't involve reading or writing anything,go for a walk or something! Then at 1pm start work again but only for another 2 hours,repeat the earlier process.Then completely stop.Again stay away from the computer-the process of typing whether it be fun or work completely distracts your mind. Do something fun but don't go back to it until the next day.The pressure to write can be immense-you need to calm down and structure your schedule so that it doesn't take over every waking minute.

Hmmm this is what I do........
Turn off the internet, go to Starbucks and sit in the corner for a few hours. I hate this stage especially when you have hundreds of pages and you need to go through bit by bit, but just take a moment to remeber that you have a great job, normal people have to go to work 9-5 everyday and a lot of people do things they don't want to to make a living....appreciate that and AMEND!
I also moan at the agent a lot...

Go out for a walk, get some fresh air, maybe you'll find some inspiration while you're out, then come home refreshed & get stuck straight into the book WITHOUT coming back to Yahoo, which will be your reward when you have finished the book.

Look, I've had one book published,and lost another to computer problems. I've had to start over with what I had on here before. Just turn the TV off, get off Yahoo for awhile, and Get er done.

Listen, kiddo, you've been getting a lot of great advice. I'm glad you've gotten so far. But, look, you on the FINAL STRETCH. You almost there. Just thinking of it as a race and you're almost there. Everyone else has basically said it. Sit down and do it. I know it gets tough when you're read your work over and over but get it down (get it down early and than celebrate) and sit back, relax, and than get started on your next one.
(A writer's work is never done.)

go for a jog, grab a cup of coffee, and make it happen. Also when i'm working on something I often get distracted thinking about other things that "I HAVE to do or else I'll forget about them and they'll never get done", I make a list of those things and leave them on the fridge.

I doubt asking for an extension will help. But you seriously need to just do it. There is no shortcut. After the 30th, you'll be finished and your book will be published.
