John Crapper being the inventor of the flush toilet is really a misconception, believe it or not. He was instrumental in its early days, and improved on its design, but was not the original inventor of it. Only his name is associated with it today, and many people mistakenly assume Crapper himself is a myth.

For the first toilet, this is probably unknown. For the first flush toilet: I do not know the exact date - it was in the 1700s. The location was London, and the inventor/engineer/presenter was Sir John Crapper. This is not a joke ... that was his name. He is buried in Westminster Abbey. Someone there might be able to get you his birth/death dates from his tomb. He is one of the few human beings to have both a noun and a verb named after him.
