Go to the local post office and fill out a form. Make sure you check the box as "individual" and write your full name. Depending on the size of your post office, sometimes you can just talk to the postmaster.

Get a PO Box. It's the best option, that way there isn't the chance that you won't get all your mail or a chance that you will think you didn't get all your mail and cause a fight over a misunderstanding.
