Manila Hotel is for me still by far the best although there are new ones that just emerged. The cost will depend on the time of the year nad other special discounts from different travel agency. You will expect to pay more or less$100 and up. Go to ManilaHotel.com you can inquire the price for the days you plan to be there, they will ask for your email.

Makati - least expensive is Dusit $70, Intercon $90
Manila Peninsula $115, Makati Shangri-la $180
Mandarin Oriental $110
Ortigas - Discovery Suites $110

Manila Hotel is one of the best I heard. It costs $200, but it really looks great. I see a lot of tourists from American and Korea there too.

If you are vacationing and want the BEST hotel - Malate Hyatt or Pan Pacific. Makati is only good if you have business to tend to there, otherwise pretty boring.
Those two hotels are probably USD 125 - 175 per night.
