It can happen, but never has. They announce the location of the Superbowl years in advance. But, of course, we don't know who will play until two weeks before. In Superbowl XIV, the LA Rams, whose home stadium was in Anaheim, played a few miles away in Pasadena. Superbowl XIX was played at Stanford stadium, not far from San Francisco (and I think the 49ers used it to practice sometimes).
Early on, it wasn't much of a problem, since they played mainly in college stadiums. It seems about 20 years ago, they changed from rotating between Tulane, Orange and Rose bowls to moving to different NFL stadiums, still heavily favoring the south (because it is warm) and large stadiums, like the Superdome.
Here's an expert opinion:
The sites for the Super Bowls are picked by the a selection comittee chosen by the NFL owners. The choosing of the sites are based on the number of hotel rooms available within a certain radius of the stadium and a suitable climate, which is why all but two Super Bowls have been played in the south or out west. Super Bowl XVI and XXVI were both played in the north (Detroit and Minneapolis, respectively). Both of those cities have domes.
One team, The Los Angeles Rams, actualy played what one might call a home game in the Rose Bowl in neighboring Pasadena, Ca. against the Steelers in Super Bowl XV.

No, through the 2011 NFL season, no team has ever played a Super Bowl in their home stadium. If this scenario were to happen, they would not move the Super Bowl to another stadium. Keep in mind, the venue for the Super Bowl is determined at least 3 years in advance.

No host city team has ever played in the game in their home stadium. The 49ers played in Super Bowl XIX, but the game was played at Stanford Stadium, not their home stadium of Candle Stick Park.

No it has never happend before and it will never happen. The Super Bowl is NOT decided in a couple years in advance. Three-Five weeks before the playoffs, they decide that the stadium of a team that has been clinched and can't make the playoffs shall be the spot of the Super Bowl. In this way, no team gets over excited that they have home-field advantage. NFL officials ALWAYS choose a neutral spot to hold the Super Bowl.

Yes they can. Super Bowl locales are determined several years in advance. Way to big of an event to wait until the last minute to schedule. Last year's SB was in Detroit, and if the Lions had gone(snicker), they would have gotten to play at home.
