I think there are better options for a gift rather than a teddy - we were given a couple when my son was born and they just get moved around, and covered in baby spit!
I am guessing you are in the US from the fact you say tehre is no mothercare anymore? As someone has said, Mothercare is still around in the Uk, if you are in the UK and there isn't one near you, you can order online.
However, why not consider either something practical like a nappy cake or a baby bouquet? (baby bouquets look like flowers but are actually made up of baby clothing)

Why is there no Mothercare anymore? We have lots here in England. Their website is www.mothercare.co.uk
The main problem is that you need to check the labelling to ensure that a teddy is suitable for a newborn baby - most specify not to be given to children under 3 - so a proper baby store is your best bet.

I am not sure where you stay but we still have 2 mothercare stores in our area however you can also buy online from mothercare but if its easier try babies r us or go to clinton cards as they sell teddys for a new baby.

As a mother to 2 kids, one of them is 1year old, avoid teddies at all cost! They just accumulate and end up getting given away! Proper baby gift boxes like the ones for christening or a piggy bank or even photo albums or frames are much more useful.

I don't know what a form is... I'm guesing you're not from the US because I'm not sure what Mothercare is either- maternity leave maybe? I'm also not sure what's all with the background story but you can buy a teddybear at a toy store.

sweet thought, but just for advice, avoid teddies - the parents always get so many, we always wind up having to give lots away... the card will be a keepsake, how 'bout a pack of diapers, or a dvd like baby mozart or something? Just a thought, good luck!
