Depends on what it is for. An affidavit is merely your attestation to something under oath, witnessed by a notary. So, you could type one up pertaining to the subject matter and then go to a notary.

Affidavits are legal forms. They can be obtained right here on the web. They can also be obtained from most office supply stores. There is a charge of course. Any law firm has affidavits.
