He was 23 and she 37 at the time. Now he's 31 and shes 44 she has two daughters from a previous relationship that are 2 and 4 years younger than him. This woman is a sweetheart, but for the sake of being nice, very unattractive. She is now a Grandma, and he has confided in having feeling of cheating on her. Not with me, but just cheating in general. Is this the natural progression of wide age gaps in men? Or is it something else? She treats him like god, and lets him have all the freedom he wants, she is faithful and good to him.

It sounds like much more a reflection on your friend than their age gap. If he has a partner that is faithful, caring, etc., and he wants to cheat on her, her age isn't the issue; his character is.

I think if he was married to a lady his own age who takes care of herself, he wouldn't be as likely to think of cheating.
It's the wide age gap, and would probably happen when the woman is much younger also.

He will be sorry when he loses somebody who loves him so much.

If he is thinking of cheating he doesn't respect her.
