Call the front desk and complain that your room hasn't been cleaned in a week!! That is unacceptable!! Also, have them transfer you to housekeeping and let her supervisor know that you want the pad changed and your own linens used. Be careful, your linens may be taken the next time your room is cleaned. I'm sure that there is a laundry room at the hotel, maybe you should just wash the linen yourself. As for a tip, I don't know why you would want to leave one? Your room hasn't been cleaned in a week!! What hotel is this? I want to make sure to steer clear of it.

go to the front desk at the hotel, and ask for the consierge and relay to him/her what you want done. Then it becomes that person's job to communicate with the housekeeping staff.
