First go to the dollar store and buy a plastic shoebox with a lid. Line it with a receiving blanket and get small things that people don't have time to go buy when they really need them: a nasal aspirator, baby nail clippers, baby Tylenol, q-tips, one of those great pacifiers that has a colored dot inside that's used as a thermometer, spare pacifiers, diaper pins (usefull for SO much more than diapers!), bibs, burp pads...
You'll give her all things she can use and no wrapping paper or stuff to throw away - I've always believed the best shower gifts are those given in containers to store the stuff in! You can also wrap the box with the receiving blanket and fasten with diaper pins for a cute look.
Also, regarding the diapers though it may be too late to coordinate, our tradition is to have a diaper drawing. Each person receives a ticket for each package of diapers they bring - can be ANY size since they'll all be used eventually anyway - and the hostess or mother or grandmother-to-be provides a gift, bouquet of flowers or gift basket for the winner of the drawing - not something baby-related.
A fun game is to get a number of baby food jars and take off the labels. Number them and pass them around, having the guests guess what the flavors are. Tasting is allowed for those brave enough and no mixed flavers (chicken and noodles, etc.) allowed - usually only fruits and vegetables...
Contact me if you need more ideas for games!

Perhaps a few outfits you'll be suprized how many clothes a baby can go through in a day. Or a nice soft blanket. Babies loved being wraped up and its something they can use for awhile. My childern are a year old and they still love thier blankets. A music box of some sort. Something to play them a lullaby at night. A baby book would also be nice, that way she can keep track of the babies development and such. So lets see clothes (if you do get clothes, get one size bigger. that way they will be able to use it longer), blanket, music box, and baby book. Heck if you want to you could go on your computer and use the painting prosses and make coupons (the only thing this will cost you is the price of paper). Now when you make the coupons it can be for anything like 'I will change one durty diper' or 'I will watch the baby so you can have a night of fun' or 'I will make dinner so you don't have to' anything you can think of that would be helpful. Make as many as you want (or think you can handle). Hope this helped. Its just a few ideas I did with my sister when she had her childern.

A great thing I like to do for baby showers is remember the Mom more than the baby. Chances are tons of people are going to get her onzies, diapers and baby toys and you can get her those things any time. Try something like a pre or post natal masage or a coupon for free babysitting or soemthing else that is for her. Of coarse you might want to throw in some cute baby stuff because it is fun to buy.

Diapers are ALWAYS needed and about $10. But you may want to go with bibs ($2), baby booties ($2), baby hat ($2), small baby sized stuffed animal ($4).
Walmart and Kmart have lotes of inexpensive baby gifts. just stroll down the baby aisle and you'll find lots under $10.
I hope that helps. Good luck!;)

A few ideas:
A Boppy Nursing Pillow ($30-35ish) is excellent, even if she doesn't nurse. I didn't end up nursing; I have twin boys and two Boppy pillows and I use them all the time. They are also good for tummy time and helping the babies sit up. http://www.shopboppy.com
You could get her a collection of the "essentials", i.e., some pacifers and pacifier holders (made by Mam -- need those holders so you don't lose ever single pacifier!!!), some burp clothes, lap pads, onesies.
You could make her a "diaper cake", which would cost about $12, and tie a $3 rattle on the top for decoration. To make "cake", buy a package of diapers. Wrap the diapers around each other to form two circular layers (the top layer smaller), and stack them on top of each other to look like a cake. Decorate with ribbons, etc. It's a cute idea that isn't very expensive at all.
A Halo Sleep Sack ($20). Endorsed my the SIDS Alliance, it is a wearable blanket for babies to avoid having lose blankets in the crib. They come in lots of colors and fabrics, such a fleece and cotton. http://www.toysrus.com/product/index.jsp?productId=2317638&cp=&sr=1&f=Taxonomy%2FTRUS%2F2255957&origkw=halo+sleep+sack&kw=halo+sleep+sack&parentPage=search
A Baby Einstein DVD ($20). Babies LOVE these, and will buy your sister some time to take a shower and get things done.

A baby bathtub ($20-$30). You could fill it with a towel and baby lotion, soap, toys, etc.
Or simply baby hooded towels and baby bath essentials (about $25)
A baby health kit (one of those little packs that include baby clippers, a thermometer, a bubl syringe, etc.)

A Diaper Genie ($20) -- indispensible for me!

This piggy bank is adorable. I used to get it for girls having babies because it was inexpensive but so chic. It has been featured in many magazines.
http://www.elegantbabygifts.com/baby-gift-4438.html
A subscription to Parenting magazine, or something similar. Probably about $12-15 if you go into a store and get one of those cards out of the magazine that offer a deal for a years subscription.
She will eventually want some toys for when the baby gets older. Walmart has really good prices for toys. You could get her some baby blocks and a ring stacker or something for under $20.
Books are always a great, great gift. I loved getting them. Some good ones are Goodnight Moon, the Runaway Bunny, Pat the Bunny, Guess How Much I Love You, etc. You could spend however much you want depending on how many books you get.
Good luck! Hopefully this helps!
I linked a bunch of things to Babies R Us so you could see what they are, but any store will sell all of these things. Sometimes you can get great deals from EBay if you have time to go through the bidding process.

One thing you can get and save money with coupons...diapers. She will need plenty of them. It's not something many think of. They do the clothing, or toys, or stuff for the babies room. Or how about a gift certificate to have pictures taken. Walmart and Meijer where I live will do that for you.

A feeding basket. purchase a large basket and fill it with magazines, snacks, water bottles, Lansinoh or olive oil if she's nursing (for sore nipples), a usual distant, a telephone charger, and so on. It on no account fails that once you're taking a seat to nurse or feed the toddler you recognize you're death of thirst or starvation or left the distant and your e book on the settee. The basket might desire to stay next to everywhere she generally sits to feed.
