It depends on what era and media you are referring to...

In the Golden Age of comics, he got the 'S' and told people he was called Superman, was not given the name.

In the Silver Age of comics AND the Christopher Reeve films, it was alternately the symbol for his family, the House of El and a stylized version of a Kryptonian symbol or letter.

In the Bronze Age of comics (John Byrne's origin) it was created by Martha and Jonathan Kent for Clark's disguise.

In the Modern Age of comics (the Birthright storyline) it is again the symbol for the House of El.

In Smallville, it is both a symbol for the House of El AND a take on the Kryptonian symbol for Hope (which is a figure 8 inside a diamond shape)

Superman Returns takes origins from the Christopher Reeve films so I assume it is still the House of El symbol.

So basically, more times than not.. it is not a 'S' insignia at all... it just happens to look like what we Earthlings assume is an 'S'

the s was from his family crest if you look in the first film all the other members of the council on krypton had different symbols. the s came with him too earth and when he had his first interview with Lois she said what does the s stand for super and that's how he got the name superman because Lois published it in the daily planet and it stuck

It was not actually the letter 'S' it was a Kryptonian symbol that just looked like the letter 'S', it was actually the family crest of his parents bearing the 'El' name.
He was dubbed 'Superman' by Lois Lane when he was first seen, because of what appeared to be a letter 'S' on his chest.

His adopted parents made his outfit out of the robes in his pod, and it was them that named him Superman. His real name is only mentioned once (in the films anyway... dont know about the comics), in the original superman with Christopher Reeve when he was a baby flying through space in the pod and his dad's voice (recorded) called him by it.

I dont know what it stood for in the original comic books, but if you go by Smallville and some of the recent stories, then the "S" was the sign of Jor-El's household. Sort of the family crest.

It's a kryptonite symbol that means 'sue' - his parents listened to the 'boy named sue' song a bit too much. country music was big on krypton. thats why the krypton factor was so called - it was actually for country music lovers.
