Depends on your position within the company. I would say most companies would like 2 weeks. However, depending on the expertise of your position and the difficulty the company may have replacing you, you should try to give more 3-4 weeks. As much notice as possible is good. You never want to burn a bridge, because you may have to use it again one day.

Give 2 weeks notice, then make sure for the last two weeks you clean up everything on your desk and make sure that someone else is trained to replace you. If no one is there to train I usually have written a short manual with examples of reports, forms or documents that I maintain. Don't spend the last two weeks on the job walking around bragging to people about how great it is to leave, how great the place you are going to is, how much it sucks to work at the place you are leaving. Trust me, you want to leave with a clean slate and your head up. Don't leave a bad impression. The world is a lot smaller than you think and you will run in to people down the road.

2 weeks notice is customary.
That is plenty of time for you to close out your projects.
Once notice is given, you make see a change in relationships. More than 2 weeks may create an uncomfortable situation.
Less than 2 weeks and managers get annoyed.

Depends upon your position and responsibilities. I would say 30 days are fair enough, that's reasonable for your employer and the future employer.

it shouldn't be less than your pay period, if you get paid every week give them a weeks notice, if you get paid every 2 weeks give them 2 weeks. That is a minimum requirement. I would give them as much notice as possible as long as you are confident they won't get upsert and just let you go.
