Honestly, it depends on what you are using the portfolio for as to what will be inside of it. When I was teaching, I used portfolios to show examples of students' work from the beginning, middle and end of the school year. That way the parents and next year's teacher could see the progress that was made by the student. It also helped me to keep track of any problems that students were having. It was full of checklists, work and progress reports (report cards).
I also have my own professional portfolio that showcases some of my best lesson plans and has pictures of my students working on projects. It also houses recommendation letters and certifications. It's a collection of my "best" work.
I've seen students create portfolios of their "best" work, too.
So, think about what the portfolio will be used for - that defines what it is exactly. :)

Portfolios are a form of assessment used by teachers and professors in order to view collected information. It is a way for teachers and professors to see that students have reached certain goals that were set for them. For example, in my teacher education course where I received my degree we had to develop portfolios which contained artifacts that showed that we met certain standards set by the institution. Also,you can do the same for school aged children by having them compile activities that they have completed into an organized notebook.

For your students it is samples of their work over the course of the year. It can also include photos and reflections on why a piece was selected to be in the portfolio.
