I read in this book that they can. The woman has to be very sexually aroused and you would have to gently squeeze the nipple for some time. But only a tiny bit of milk will come out. Probably less that a drop. I can't full remember how to exactly do this, i read it a long time ago.

Yes, with massive amounts of certain hormones. Some women will do this under a doctor's supervision if they are adopting a baby, but still want to breastfeed it.
Whether this is safe or good---I'm not sure, but I had a friend who did it years ago when she adopted. It was not easy for her, but she did manage to produce some milk.

Yes, but it is uncommon, and it generally requires "training" the breasts. Also, it usually works best if this training begins after the woman has had a child.. you just prolong the whole process.
