You need to get on a good and regular skin regime. This is not a quick fix, but a lifestyle like a good diet (which also helps your skin by the way).
1. Exfoliate Twice a week.
2. Use a ph balancing facial cleanser.
3. Always use COLD water! This keeps your pores shut tight which will prevent stuff getting in, in the first place.
4. Use a ph balancing toner after you wash with a cotton ball.
5. Use a good moisturizer that is not oil based. Gel facial moisturizers work best.
6. Do not use a make-up that is oil based or clogs pores.
7. Keep hands away from your face. Do not pick, if you must express, do so with tissue wrapped around your fingers and apply GENTLE pressure. Discard after each blemish so as not to transfer infection or bacteria and re-wash hands and face afterwards. If it won't come out...IT"S NOT READY! LLEAVE IT ALONE!!!
8. Bio-elements has a refiner that will dry them up, but not cause flaking and should be used after toner and before applying make-up.

Few things that have worked for me:

Only washing my face with cold water

using Lush MudFlats soap combined with their Coalface soap.
I use one at night and one during the day.
Now I am a bit older than you but I wish someone would have told me about Glycolic! Now there are many forms of it but I would suggest asking your local pharmacy what they can get for you. Just to see if it will work for you.

I'd also like to add that when it comes to skin less is more. Overdoing it will just lead to more problems.
