I would type up a letter and tell he/she that you believe you are eligible for a raise because... List all the reasons that you know of that are beneficial.

Not a good idea. Your letter may become a prop for a laugh around the office.
Do a better job than you are expected to do, be nice, don't complain, ask how you can be considered for a more responsible job with the company after setting the tone by achieving. Don't try to excel by tearing anyone else down. If you are outperforming someone else, let your bosses notice that if they will, don't call it to their attention. You will get a raise or a new job most likely. If all else fails find a new job at better pay and give your existing boss the courtesy of a two week notice verbally at first. If they want to retain you, tell them how much you will stay for, if you can't work that out, give them your notice in writing and take the new job.

You ask for the raise and then build your case point by point as to why you should get a raise. It is then in your bosses' hands and you will get an answer and some direction if it is not granted. If you don't like the answer start looking around. We think we are worth more but only the company sets our worth.

It all depends on when your last review was. Send an email to your boss saying that it has been some considerable time since your last review and that in the mean time your skill sets have grown as well as your duties to the company (thus the indirect way of asking for a raise)

ok... The best way to get a raise is to show the company why you deserve it (rather than, "i have worked here a year and my kid needs braces").
If you can quantify how you have improved things or made them money then a raise is easy.
Something like "Since i commenced here i have improved process X by X% and this has resulted in a cost saving to $x to my department"
I just made that up, but you get the idea. Show them why you are valuable to them and worthy of a reward for that.

Ask in person. Start the conversation with how long you have been with the company, what you have done for the company to make yourself an asset and simply state that you believe you deserve a little extra wages for the time and effort you have put into the company.

I would say that you should open it with a self review of the past XX months highlighting your accomplishments/achievements and how you have exceeded the scope of your position. By doing this, you will make it hard for them to say "no" to your request for a raise.

eye-2-eye is best - especially if you have figured out why you deserve a raise.
List all the things that you have done that make your job more important TO THE COMPANY (emphasis) since you were assigned to it.
Are you generating more business for company; are you working 3 positions because they outsized some colleagues and shovelled all on your plate?
Be Tactful - tho.
