Typically 401(k) plans are run by an investment house and they will have a toll free number to call. Somewhere you have paperwork around for them..

If not, then call the main office of your company or former employer. Ask to speak to the retirement or benefits person. They will tell you a website or phone number.

Be advised that there are unpleasant tax liabilities for an early withdrawal of retirement plans.
