Epithet is the word.

I think you are thinking of the word epitaph, which is a short poem or saying usually on a grave commemorating the deceased.

here is an epitaph written by alexander pope to a deceased friend

Of Manners gentle, of Affections mild;
In Wit, a Man; Simplicity, a Child.

You could also mean an epithet, which is any word used to describe a quality of a person (traditionally used to mean both positive and negative associations like Richard the Lionhearted or Ethelred the Unready, now the word usually is used to mean pejorative statements, like racial epithets... I am sure you can think of many of them).

hope that helps
