Poison arrow frog and certain salamanders
Box Jellyfish, Stonefish, Some Spiders, Some Snakes (Inland taipan) and so on and so forth. No easy answer here, not really

This is not so easy to answer -- because dead is dead -- no matter how many times over!
Coral Snakes have the fastest working venom.
The most "poisonous" is the poison arrow frog
The most "venomous" is the Box jelly fish.

I would say it's the mosquito.
While it doesn't produce it's own venom, poison or disease, they are the cause of the world wide spread of malaria, st. louis enciphylits, avian bird flu, and a few other diseases which have led to millions of deaths.
