personally id do neither, her hand all horrible and wrinkly and everytime i kiss her cheek i get a static discharge off her beard
so i just tend to hug here :D

Pray tell Sir, is One's Grandmother the Queen? You should really stop trawling the internet Prince, you already have quite enough hot biscuit at home looking at the papers. Who'd have their hand kissed in this day and age? You should give her a warm hug because she's your gran, not kiss her like a servant.
Unless she is the Queen, in which case, I'm sure there's a protocol.

cheek, if good explanation about kissing her hand and good reason to make it so its not gonna happen and explain how good of friend or how good you will be by not saying so and without kissing her on the cheek then the lips. id vote the cheek though.
if its a gf then the cheek or lips

I absolutely adored and loved my grandmother Catherine and so did my brother, she brought us up until we were about nine years old. After that when ever I saw her I hugged and kissed her, she was only about 4`11" tall, and my 6`3" tall brother, used to stretch out both of his arms to her, pick her up way above his shoulders, twirl her around and say " Whats` to eat Nan ?

Follow her lead or just give her a little haug and if your comfortable when you say goodbye give her a kiss on the cheek ( it'll let her know you love her and care about her, even if you don't see her a lot!)
