According to my Spanish instructor, ¿Comó esta usted? is the way to ask in a formal manner, such as asking an instructor, or someone who you are not real close to. ¿Comó esta? or ¿Que tal? (What's up??) is appropriate when addressing friends or close family members.

Bien, pero tengo un dolor en la cabeza ahora. (Geeze, I didn't realize you were asking how to say it! I thought you just wanted to know. Sorry.)
